What is an Independent Medical Examination?
An Independent Medical Examination, or IME, is a legal and medical evaluation. It helps determine the condition of a worker after an injury. Either the Department of Labor and Industries (L&I) or a self-insured employer usually requests the IME. Only providers approved by L&I may perform these exams.
The IME offers an unbiased medical opinion. The goal is to understand the injury, its effects, and what treatment the worker needs. The examiner reviews medical history, performs a physical exam, and provides a detailed report. This report includes conclusions about the worker’s current condition. The examiner focuses on medical facts, not opinions. They base their findings on records, exams, and clinical judgment. Their report may influence decisions about future treatment, benefits, or work ability.
The IME does not replace your regular doctor. It does not offer treatment or advice. Instead, it answers specific questions about your injury. It helps resolve disputes between doctors, patients, and insurers.
When Do You Need an IME?
Usually, L&I or the self-insurer requests an IME because it disagrees with a decision by the treating doctor about the course of medical treatment or the extent of any permanent disability. For example, they may question a diagnosis, treatment plan, or permanent disability rating. When this happens, they request an IME to get a second opinion.
Another example is when auto insurer is deciding how to pay out for an auto accident. You may need a second opinion of the medical issues sustained from a car wreck and how long it will take to recover.
The IME helps determine whether your treatment is appropriate and effective. It can confirm if the condition still relates to the original workplace injury. It also evaluates whether your recovery has reached a stable point. Sometimes, the examiner will assess whether you have any permanent impairments. If so, they measure the degree of impairment. This helps guide decisions about long-term benefits or vocational support.
An IME also helps decide whether you can return to work. The examiner may outline your physical restrictions or ability to perform specific tasks. This information helps employers and case managers create safe return-to-work plans.
